How to Get Your Voter ID Card?

Fill Form 6 - New Voter / Change of Assembly Constituency

Fill Form 8A - Change of Address within the same Assembly Constituency

Proof of Age (if you are between 18 to 21): Birth Certificate issued by the Municipal Authorities Proof of Residence:
	a) Bank / Kisan / Post Office current Pass Book, or b) Applicant's Ration Card / Passport / Driving License / Income Tax Return filed or Assessment Order, or c) Latest Water / Telephone / Electricity / Gas Connection Bill for that address, either in the name of the applicant or that of his / her immediate relation like parents etc. or d) Postal department's post received / delivered in the applicant's name at the given address. Students living in Hostel/PG need to fill a Declaration (click here) & attach with Form 6.

Document Upload is Optional & can be given to Booth Level Officer (BLO) during their Verification visit.

Nothing additional is to be done to cancel the previous Voter ID card.
